In 1976, he rejoined the secret organizations of the Kurdistan Workers Party (PKK) in the Rzgari Organization of Branch 1. In 1980, he returned to the ranks of the Peshmerga and served in the Martyr Khadr Mahmoud Organization.

In 2000, he was appointed commander of the 7th Force of the 18th Army.
On August 14, 1998, while removing a mine in the football field of Betkar village in Duhok province, he lost his left eye and hearing. He participated in 25 battles and was wounded seven times.
In September 1986, two brothers, two nephews and a son of Ismail Betkari were martyred by Iraqi warplanes. Another brother, Izzat, was martyred in December of the same year. His father, Sadiq Betkari, was a Peshmerga The September Revolution He was poisoned in 1993 in the Mosh refugee camp.
Ismail Betkari retired with the rank of lieutenant due to his disability and now lives in Duhok 13th Congress of the Kurdistan Democratic Party by the President Massoud Barzani He was awarded the Barzani Medal.
