Khalid Azam Tofiq was born in 1960 in the village of Barwen, Bamo sub-district, Halabja province. He was a villager and studied up to the primary level.

In 1976, he became a Peshmerga in the Martyr Aziz Tofiq Abdulqadir battalion, which was affiliated with the Tagozi battalion, and Qadir Hama Salih was the battalion commander. In 1978, between the village of Sela and the village of Pesta in the Bamo sub-district, they fell into a military ambush by the then-Iraqi regime, and Khalid Azam Tofiq was severely wounded.

On September 10, 1982, Khalid Azam Tofiq and a detachment of comrades, under the command of Aziz Tofiq Abdulqadir, after returning from a partisan activity in the city of Halabja, in the village of Gurgachia in the Tagozi region, Halabja province, a large force of the Ba'ath regime's Jash and soldiers surrounded the village, and a battle broke out between them. After a few hours of confrontation and due to the small number of Peshmerga and the large number of the regime's soldiers and Jash, Khalid Azam Tofiq and his commander, Aziz Tofiq Abdulqadir, who was his uncle, were captured while wounded by the soldiers. Later, by order of the Iraqi General Intelligence, they were executed by firing squad on the night of September 10, 1982, at the entrance of Gurgachia and were martyred.
